Fall of roof in the main engine road at a bridge (about 2 yards in length), or short piece left unfinished by the repairers. The timbers were probably loosened by a journey which got off the road a quarter of an hour before. The examiner of the district examined it and thought it safe and was at the place when the accident occurred. I think he was guilty of an error of judgement in allowing the journey and the men to pass, although he had had some timbers set to secure it.
