      - Details of Event

      - Deceased was attending to an enginewhich was arranged for hauling and pumping. The pumping gear only was at work at the time of the accident. He, presumably - no one saw the accident occur - had been pouring melted grease into a bearing situated between a fly wheel and a rope drive pinion wheel, when his clothing had been caught and he thrown violently to the ground and killed. The bearing he was attending to had was provided with a proper lubricator, but this had been broken and not replaced. Neither the fly wheel nor the rope drive pinion wheel was guarded by fencing.
