      - Deceased was wedging down coal at a buttock in the Top Coal seam when a fall of roof rectangular in shape and 9ft 6ins. by 5ft. 6ins., occurred from four slips all converging upwards. The outer edge of the fall extended about 2ft. beyond the buttock and struck deceased down and killed him instantly. Deceased was wedging the coal down to make room for the next face post to be set 4ft. 6ins. or 5ft. from the last one.
