Details of Event

Deceased was engaged in repairing the water garlands in a pumping shaft. He stood upon a small scaffold - two cross beams and a plank - when one of the cross beams became displaced and deceased was precipitated down the shaft a distance of 34 yards, into 11 yards depth of water, and killed. Apparently the cross beam hat not been securely fastened in the side holes in the shaft.
