Details of Event

His duty was to lower full waggons of small coal down to and empty them into a hopper. The hopper was covered with an iron grating but for some reason a portion of the grating had been removed without the knowledge of the officials. Deceased while dropping the bottom door of a waggon fell into the hopper and was suffocated. If the grating had been in its proper position the accident could not have happened.
