Details of Event

Deceased and another workman had made preparations to descend a shaft used for pumping water. Ordinarily the top of the shaft was completely covered level with the surface by cast-iron plates. One of these had been removed leaving a rectangular opening 2ft 7ins. by 2ft. 1in. Deceased walking towards the shaft stepped into the opening and fell down the shaft. There was nothing to account for his doing so.
